NEW IMMUNIZATION REQUIREMENTS
FOR 2010/2011
ARE AS FOLLOWS:

The Indiana State Department of Health has issued new requirements for the 2010/2011 school year.  Under Indiana Code 4-22-2 ALL school age children are required to receive additional immunizations against the following:
            - Meningitis (MCV4)
            - Pertussis (whooping cough) (Tdap)
            - 2nd Varicella if your child received the 1st Varicella vaccine.
 (If your child has a history of having had the chicken pox disease then this vaccine is not needed. However, a doctor's signature assuring a documented history of Chicken Pox must be submitted to the school nurse in order to satisfy the Varicella immunization requirement.)

Please make an appointment with your child’s doctor, or with the Knox County Immunization Clinic (at 882-8700) to have these immunizations taken care of now.  These immunizations are a requirement for enrollment in this school year.   If you have any questions contact Nurse Sara at 882-2137.

If your child receives immunizations at a physician’s office, you MUST provide the nurse with written documentation from the Dr.

**Students are being given a TWO WEEK grace period in which to get the now required new immunizations. You must contact Nurse Sara with an appointment date if you have not received these immunizations OR face expulsion from school. Notices have been going out since April, 2010.
